Getting Started
Online Intake Paperwork
New patient intake paperwork must be completed before we can schedule an appointment.
Once your paperwork has been submitted, our front office team will verify your insurance benefits and contact you to schedule an evaluation or add you to our waitlist if no appointments are currently available.

If your child has an IEP through school, bring a copy with you to the first appointment or have it faxed to our front office!
If your coming to our practice for learning with an AAC Device, bring that along so we can work with it together!
If your coming to our practice for feeding/swallowing therapy, bring along some preferred and non-preferred food items!

During your initial evaluation, you’ll work one on one with one of our licensed and highly skilled speech-language pathologists to go through different tests and get an idea of what’s going on.

After your evaluation, your speech therapist will write up your evaluation report to send to your insurance company so that we can request authorization for treatment visits.
Once we receive authorization from your insurance company, we’ll give you a call to get you set up for weekly treatment visits so that we can get to work!
